This policy applies to personal information we collect through our website, over the phone, by email, and in the course of providing aged care and support services, including services delivered under the Support at Home program.

Depending on our relationship with you, we may collect: your name, contact details, date of birth and Medicare number; My Aged Care reference and assessment information; health, medical and care-related information; emergency contact and next-of-kin details; financial information relevant to your care package or fees; and information you provide to us through enquiry forms, phone calls or emails.

Some of this information is sensitive information (such as health information) under the Privacy Act, which we handle with additional care and only collect with your consent or as otherwise permitted by law.

We generally collect personal information directly from you — through our website enquiry forms, by phone, email, or in person during assessments and the provision of care. We may also collect information from My Aged Care, your representatives, healthcare providers, or other government agencies where necessary and permitted by law.

We collect and use your personal information to: assess your eligibility for and coordinate care services; provide, manage and improve our services; communicate with you about your care, enquiries and our services; meet our legal, regulatory and reporting obligations, including under the Support at Home program; and for administrative and billing purposes.

We may disclose your personal information to: our staff and contracted care workers involved in delivering your services; My Aged Care and relevant government departments; your nominated representatives, family members or emergency contacts; healthcare providers involved in your care; and our professional advisers and service providers (such as IT and software providers) who assist us in operating our business, on a confidential basis.

We do not sell your personal information to third parties.

7. Storage and security

We take reasonable steps to protect the personal information we hold from misuse, interference, loss, and unauthorised access, modification or disclosure. This includes secure electronic storage systems, restricted staff access, and secure disposal of records that are no longer required.

You have the right to request access to the personal information we hold about you, and to request correction of any information that is inaccurate, out of date or incomplete.
